Our mission is to assist our clients in reaching their maximum functional and rehabilitative potential. We will consistently set obtainable goals for clients and encourage our clients to meet their goals. Our professionals will educate clients in terms that are understandable and will answer all questions relating to the health of the client in a factual matter.
Our priority as professionals is to act as a patient advocate in all situations.
Our nursing staff is trained and assigned to cases based on their level of experience and competency. Our nursing staff provides the following services: wound care treatments, injection and intravenous therapy services, medication management, bowel care programs, bladder care programs, Gastric and Nasogastic tube feedings, TPN, Dialysis teaching and assessment, Chronic disease management, laboratory blood draws and specimen collection, diabetic management, ventilator care, ostomy care: among others.
Physical Therapists work with the patients to make sure that the home environment is safe, they teach fall prevention, and request equipment if necessary to improve the safety of all clients. Our therapists will set individual goals at the start of care and will work together with clients to meet their goals. Therapy goals may be for strength, endurance, balance, coordination, gait training, transfer training, among others.
Social Workers provide case management and community resource information. Medical Social Workers will find assistance programs, complete applications, and will work side by side clients, caregivers, and providers to establish clear lines of communication. Social Workers will also work together with families who live out of the area to provide communication and coordination support.
Occupational Therapists work closely with patients to decrease functional limitations and possibly recommend assistance devices if needed.
Home Health Aides assist with bathing, dressing, shaving, nail care, hair care, skin care, foot care, and all personal hygiene measures. They can assist with light housekeeping, meal preparation, laundry, and shopping.
Companion Services assist with meal preparation, light housekeeping, laundry, shopping, and home maintenance. A companion may also read, play cards, interact with clients in other such ways to keep the client company.
Live-In attendant 24 hours a day –May assist the client with all activities of daily living and chores that the client is unable to perform alone. Attendant will also assist with meals, home maintenance, light housekeeping, and client safety. Activities may also include assistance with self-medication administration and transportation. The plan will be established upon admission to the agency.
